Another part of BMS Access Support is assisting you and your patient with identifying potential infusion providers for your patient that may be close to where your patient lives or works. Once an infusion provider is chosen, BMS Access Support can contact the infusion provider to determine if the patient’s insurance will be accepted by that provider. BMS Access Support will also follow up with the patient, the chosen infusion provider, and physician to confirm that the patient has obtained access to initiate therapy.

Identify Potential Infusion Providers:

  • Use our Form Wizard (available on desktop) or download the enrollment form on your computer. Fax an enrollment form signed by the patient and physician to 1-888-776-2370. Check the box for the requested Identification of Potential Infusion Providers service(s).
