A Prior Authorization (PA) is a verification from your doctor that states that your medication is medically necessary. A PA is not necessary in all cases. - Claims Appeals
Almost all health insurers have a special process to appeal a coverage decision. BMS Access Support can provide information about this process. In this case, you and/or your doctor would prepare paperwork to support the appeal.
